Please be sure to bring your patient registration card and health insurance card.

Reception hours are from 8:00 a.m. Please swipe your magnetic patient registration card through the Check-in Machine for Returning Patients before or during your scheduled appointment time slot.

Please note that if you enter after the scheduled time slot, you will be considered late.
