Overview
"Admission" summarizes the preparations and procedures for your admission and stay at our hospital.
Admission procedures
- Patients on the waiting list after their admission appointment will be contacted by the hospital at a later date. We ask for your patience.
- On the day of your admission, please complete admission procedures at the Medical Affairs Department Admission/Discharge Reception (Counter 20) by 10 AM.
Documents, etc. required for admission procedures
Patient registration card and various medical certificates (health insurance card, health insurance card for elderly in the latter stage of life, Certification of Pollution-related Health Damage, Eligibility Certificate for Child/Infant Medical Expenses Subsidy, Reduction of the Standard Amount of Patient Liability Certificate, Eligibility Certificate for Ceiling-Amount Application, medical card ticket)
*Please submit your health insurance card to the ward clerk in case of emergency admission.
*Please also bring your Resident Card. Visitors to Japan are required to bring their passports.

Caution:
- Keep valuables and cash to a minimum!
- No bedding preparation is needed.
- Necessary items for each medical department will be explained to you prior to admission.
- Compatible with the electronic medication notebook system "harmo".
Attendants
- As a rule, attendants are not needed in general wards.
- If a family member wishes to attend an infant or a patient with a medical condition, please consult an attending doctor or a nurse. Please note that an Attendance Certificate cannot be issued.
Meals
- Meals during admission are managed by a dietitian and served at the right temperature and at the right time in accordance with the "Standards for Special Management as Established by the Minister of Health, Labour and Welfare". For regular meals, you can choose between Meal A and Meal B for breakfast and dinner.
- The co-payment for meals during admission is 460 yen per meal. However, a reduced amount will apply if you present a discount certificate issued by the municipal authorities at Counter 20 during your admission procedures.

If household is exempt from municipal residential tax and receiving old-age welfare pension: 100 yen
*In case of medical treatment without health insurance, the fee is 640 yen per meal.

- Meals are an important part of your treatment. During your hospital stay, please eat the meals prepared by the hospital. Other meals and snacks may interfere with your treatment, so please consult your attending doctor or nurse first.
Your stay during admission
As a safety measure for medical care, patients are asked to wear an inpatient ID wristband with their name written on it. You also may be asked to state your name again when undergoing tests, receiving injections, etc. We ask that you not only answer when we call your name but also proactively cooperate in our "Give Your Own Name Campaign”.
- Any behavior during admission that disturbs other patients, disrupts the order in the hospital, interferes with treatment or nursing care, or is disruptive to hospital staff, such as abusive language or violence, leaving the hospital without permission, and eating, drinking, or smoking on the premises is strictly prohibited. If such behavior is discovered, it may be reported to the police and steps may be taken to forcibly remove the patient from the hospital.
- Alcoholic beverages (including low-alcohol or alcohol-free) and smoking are prohibited in the hospital and on the premises. (Please also refrain from smoking outside the hospital premises, as it is a nuisance to neighbors.)
- Please do not leave the ward without permission, as leaving the ward without permission interferes with your medical treatment.
- As the use of cell phones or Personal Handy-phone Systems in the hospital may affect medical equipment, etc., please use them in the designated areas.
- Please do not make loud noises or raise your voice in hospital rooms. Please use earphones (headphones) when listening to TV or radio.
- Outpatient consultations are not possible on the day of your discharge.
- Please refrain from bringing in fresh flowers due to infection control measures.
- There are various restrictions on receiving medical treatment covered by health insurance at other hospitals during admission. Please be sure to consult your attending doctor or nurse in advance.
- Permission from your attending doctor is required to leave the hospital, even for a short period of time. If you wish to go out, please notify your doctor or nurse and submit an "Application for Temporary Leave". Leaving the hospital without permission is prohibited.
- In the event of an accident or a disaster, please follow the guidance of the staff and act calmly. Each patient room has an evacuation route map. Please check this after your admission procedure.
- We do not take phone calls for patients.
- Please refrain from bringing electrical appliances into hospital rooms.
- If equipment or other items are damaged, you may be asked to pay for the damages.
- Ordering items for yourself and having them sent to the hospital is prohibited.
Theft prevention
- This hospital does not hold money or valuables on behalf of patients. Note 1: Please be aware that the hospital cannot be held responsible for any robbery of such items.
Note 2: In the event of losing something, please inform a nurse.
Note 3: Please take caution as the number of thefts targeting hospitals has been increasing.
Medical consultation
If you have any concerns or worries about admission expenses, matters after discharge from the hospital, or some other issue, please contact the Medical Welfare Consultation Office, Counter 21, where a medical social worker will assist you. For patients who have difficulty walking, please notify a nurse to have someone come to your ward to assist you.
Other
- A nurse will explain about your schedule during admission. If you have any questions, please do not hesitate to ask.
- Patients or family members who wish to have their medical condition explained by the attending doctor in person should consult with a nurse in advance.
